Dulwich College (Singapore) is committed to the safety and well-being of every student.
Kindly complete the Medical Consent Form and upload your child’s immunisation records via the checklist in the Admissions Portal by 20 November 2025.
For children entering Toddler and Nursery, families must upload copies of immunication records that meet Singapore's national immunisation requirements. You may refer to the full schedule here: National Childhood Immunisation Schedule - Singapore
Please note that your child cannot begin school until both the form and the updated immunisation records have been submitted.
Immunisation Requirements
In Singapore, immunisation for Diptheria and Measles are compulsory by law. For more information on the National Childhood Immunisation Schedule requirements in Singapore, please click here.
Foreign-born children aged 12 years and below who are first-time applicants for any of the following passes:
- Dependent's Pass (DP) or Long Term Visit Pass (LTVP) issued by the Ministry of Manpower (MOM),
or - Student's Pass (STP) issued by the Immigration and Checkpoints Authority (ICA),
must submit documentary proof of vaccination for diphtheria and measles to the Health Promotion Board (HPB) for verification before applying to MOM or ICA.
This is a mandatory requirement for for their long-term stay in Singapore.
